Description of project:
All hourly, half day projects: Water must be provided. All 8hr day projects: Meal and Water must be provided. I agree to the terms and conditions, I have read and returned the agreement to the agency via "forward" email as a confirmation of booking along with this form. Without terms & conditions agreement being returned to agency, the project booking can not begin. Payment is as agreed between booking agent and booking client, $ total project. I have read and understand the cancellation policy and payment terms.
